often the firefighters call the fire department a second family what is the meaning or history behind this?
Firefighting forms a bond between the members within a fire department. These are the people that you go through the good, the bad, and the ugly with. If you have a bad or a stressful run these are the people that get you through it. You spend countless hours with these people, and they basically become you’re family away from home. Even if there’s somebody you don’t like within your department, if they really needed you, you would be there because that’s you’re family. It’s something that is very hard to put into words, it’s just something you would have to experience to understand it.

If the home owner is gone, and a neighbor reports the house on fire, what will the fire department do to gain access to a "fortress" type home that has a security gate and highly reinforced fencing around the property?
Most times, a homeowner like this has given the PD and the FD access codes to the gate.
Also, I would assume the homeowner has great security, which would generate a call or signal to their security system, who would then dispatch the FD with appropriate information.
This video was complied from files in the Stillman Fire Collection, Perlinger Archives and put together to make this presentation represent how a huge building fire might have been fought in Boston, Massachusetts in the early 1930s. Note for general interest and comments that at 2:16 a relatively new American LaFrance pumper has a hand-crank siren and a Roto Ray warning light. However at 3:03 a similar ALF is shown with an electo-mechanical siren. Obviously, I added the sound effects to this video, but the original film was silent. If you wish to see the film as it was made you will need to turn your computer speakers off.

When a smoke detector starts sounding an alarm, is the fire department notified? If so, how long does it have to sound the alarm for the fire department to take an action?
An ordinary smoke detector does not notify the FD. If you have a monitored security system, it usually includes smoke detectors that alert the monitoring company, which in turn can call the police and FD.
